WHAT YOU SHOULD BRING TO YOUR APPOINTMENT
  • Medical and X-ray Forms with Part A completed
    (these forms can be obtained from http://www.immi.gov.au/allforms/)
  • Passport
  • 2 recent passport photos (1 photo only for children under 11 years)
  • Spectacles or contact lenses if you wear them
  • Copies of specialist reports, if available, for any major medical illness

Please note that you should have the following information to hand:

  • Your visa class
  • The visa office where you have lodged your application
  • The visa office where you would like your medical results despatched
