• Completed Section A Medical Form or Appendix D if sponsored
• Passport
• 2 recent passport photos for adults completing Section A Medical Form (1 photo for children aged 5-10 years), and 4 recent passport photos for adults completing Appendix D (3 photos for children aged 5-10 years, and 2 photos for children below 5 years)
• Spectacles or contact lenses if worn
• Copies of specialist reports for any major medical illness
